Cutting Salt Was the Wrong Advice

Decades of dietary guidance told us to avoid salt like poison. But a landmark study rewrote the rulebook, and most people still haven't heard about it.

If you're over 40 in India, chances are you've heard this. Maybe your doctor said it to you. Maybe your mother heard it. Maybe it was plastered on a health pamphlet in some waiting room with outdated magazines. The advice has been around so long it feels like common sense, like saying "don't smoke" or "exercise more." Salt is bad. Salt causes high blood pressure. High blood pressure causes heart disease. Therefore, cut the salt.

Except, and this is the part that should make you sit up, that chain of reasoning was built on a foundation that's been quietly crumbling for years. And in 2017, one of medicine's most prestigious journals published a study so large, so thorough, and so inconvenient for conventional wisdom that cardiologists around the world had to pause and reconsider what they thought they knew.

The finding? Low-salt diets may actually kill more people than high-salt ones.

The Study That Changed Everything

Let's talk about the research first, because it deserves its full weight.

This wasn't a small study conducted over a few months on a handful of volunteers. This was a massive, multi-country, nearly-decade-long investigation into the real-world relationship between sodium and heart health. And what it found fundamentally upended the simple story we'd been told.

Restricting salt too aggressively didn't protect the heart. It stressed it. People eating very low sodium weren't the healthiest group; they were among the most at risk.

To be clear: this doesn't mean you should empty an entire salt shaker into your dal every morning. The study is specific. Extremely high sodium, above 5 grams a day, does raise risk, but only in people who are already hypertensive. For the vast majority of Indians who eat in their traditional patterns, salt isn't the problem.


So Where Did "No Salt" Even Come From?

Here's the uncomfortable truth about medical guidelines: they are often decades behind the science that produces them. Updating a guideline is slow, political, and institutionally difficult. And the "salt is evil" narrative has roots so deep that even well-meaning doctors still repeat it reflexively.

The story begins in the 1970s. Researchers were studying a very specific population, people with severe, extreme hypertension, the kind that would now be treated aggressively with multiple medications. In this narrow group, cutting sodium did lower blood pressure. The finding was real. The problem was in what happened next.



Imagine designing traffic laws based exclusively on studying people who'd already had accidents, and then applying those laws to every driver regardless of skill or circumstance. That's roughly what happened with salt.

A generation of doctors trained on this dogma. It became embedded in textbooks, printed on hospital pamphlets and repeated in every routine check-up. And like most powerful myths, it contained just enough truth to feel entirely true, even when the full picture was far more complicated.


The Real Villain: The Sodium–Potassium Ratio

Here's the part most people have never heard, and it changes everything.

The human heart doesn't run on sodium alone. It runs on a balance, a precise, lifelong negotiation between sodium and potassium. These two electrolytes work together to regulate blood pressure, fluid balance, nerve signals, and muscle contractions. When the ratio between them tilts too far in one direction, the system strains.

The problem for Indians, and for most of the world frankly, isn't that we eat too much salt.

It's because we eat too little potassium.

Think about what a traditional Indian plate once looked like: coconut water, leafy greens like palak, bananas, lentils, and seasonal vegetables. These are potassium-dense foods. But as urbanisation accelerated and processed foods replaced home cooking, potassium intake quietly collapsed while sodium stayed roughly the same.

So the blood pressure numbers crept up. And doctors, seeing the elevated pressure, landed on the most intuitive explanation: salt. Cut the salt. But the real deficiency, the one silently damaging the heart, was potassium.

Stop Obsessing Over Salt. Start Tracking Potassium.

If you take one practical thing from this piece, let it be this: most of us need to eat more potassium, not less salt.

The target is 3,500mg of potassium per day. That's the number most health bodies now point to as optimal for cardiovascular protection. Here's the sobering reality check:

Average daily potassium intake among Indians:- 1,500 mg

That's less than half the recommended 3,500mg, and it's the real deficiency quietly harming your heart.

The good news is that fixing this doesn't require exotic supplements or expensive diet overhauls. These foods are common, affordable, and deeply embedded in Indian cuisine. We just stopped eating enough of them.


What This Means for You, Practically

If you're someone who was told "no salt" by a doctor, I want to be careful here: don't throw that advice out the window without a conversation. If you have severe, uncontrolled hypertension, the kind the original 1970s studies were examining, then sodium restriction may still be appropriate for you. Your doctor knows your individual situation in ways a blog post cannot.

But if you're a generally healthy person over 40, eating a reasonably traditional Indian diet, with blood pressure in the normal or mildly elevated range, the evidence suggests you're not the person this advice was designed for. You're in the 4–6g/day zone that the Lancet study identified as safe. And you're almost certainly not eating enough potassium.

There's also a quieter argument worth making here about the psychology of dietary advice. When we're told to cut something, to eliminate it, avoid it, or feel guilty about it, we don't always become healthier. We become anxious. We stop eating traditional foods that were actually fine. We replace them with processed "low-sodium" alternatives that are often worse in every other way. The obsession itself becomes a kind of damage.

Eating a pinch of salt in your dal isn't what gave your father a heart attack. The lack of palak, the switch from coconut water to packaged juice, the disappearance of bananas from the daily diet: these are far more likely culprits. And they're also far more solvable.

The Bigger Picture

The salt story is, in a sense, a story about how science actually works: messy, slow, prone to institutional inertia, and always more complicated than the headlines suggest. The "salt is evil" guideline wasn't invented by bad people or corrupt scientists. It came from a genuine finding, applied too broadly, for too long, without enough willingness to challenge it.

That's not a reason to distrust medicine. It's a reason to stay curious about it. To ask follow-up questions. To look for the nuance that single-line advice inevitably strips away.

The Lancet didn't just publish a study about sodium. It published a study about the danger of oversimplification, of taking a complex system like the human body and reducing it to a single, manageable villain.

Salt was never the villain. The story was always more interesting than that.
